Yes... it was the landowner. The trap-doors failed to close after the kibble came up, and he stepped back...

I can only second lozz' post, particularly in the light of having a direct professional connection with the whole issue of long-term stability of old workings.

Those big Welsh caverns are scary places. If I understand the matter, there is only one entry to one of the main setts? I wouldn't go in there for all the tea in China, if so....

There's something on the web, according to the Mines Inspectorate the person concerned had been working on the site for 10 years, mucking out the shaft with a kibble powered by a diesel winch, leaning out to grab the kibble in order to land it, no safety trap doors or toe boards, no safety harness.
