I was reading a bit about this elsewhere. It would seem there are no shafts in this mine so how he died is a mystery unless it's related to diving.

I think one has to consider the meaning of the word "shaft" to the general media.

It could be a bit of a hollow surrounded by brickwork. Its about their knowledge level. More likely an adit

I checked out the Coal authority map a few hours ago and all the likely looking places are marked as 'Adit'.
